The following insurance networks are currently accepted. Coverage is not a guarantee of payment; benefits and eligibility should be verified directly with your insurer.
Before scheduling, ask the member-services number on the back of your insurance card whether Adam Lukeman, LCSW is in-network under your exact behavioral-health plan.
Private pay
Cash fees without insurance
Private-pay fees apply when insurance is not being used or when a service is not covered by your plan.

A sliding-fee scale may be available.
A limited number of reduced-fee spaces may be offered based on financial need and current availability. You are welcome to ask about the sliding-fee scale during your consultation. Eligibility and availability are determined individually and are not guaranteed.
Using out-of-network benefits
Your plan may reimburse part of the session fee.
If Adam is not in-network with your insurance, your plan may still include out-of-network mental-health benefits. You pay the full session fee directly, and Adam can provide a superbill containing the information typically needed to request reimbursement from your insurer.
Reimbursement is determined entirely by your insurance plan. A superbill does not guarantee that a claim will be approved or establish the amount you will receive.
Call your insurer
Confirm that your plan includes out-of-network outpatient mental-health coverage before beginning treatment.
Attend and pay
The full private-pay fee is due according to the practice payment policy.
Receive a superbill
Adam can provide a statement showing the service, fee, diagnosis, and provider information needed for claim submission.
Submit your claim
Follow your insurer’s instructions and keep copies of the superbill, claim form, and reimbursement decision.

- Does my plan include out-of-network outpatient mental health benefits?
- What is my out-of-network deductible, and how much of it have I met?
- After the deductible, what percentage of the allowed amount will the plan reimburse?
- Is authorization or a referral required before treatment begins?
- How do I submit a superbill or member claim, and is there a filing deadline?
